CARBAMAZEPINE - CHEWABLE TABLETS - ORAL
(kar-bam-AZZ-eh-peen)
COMMON BRAND NAME(S): Tegretol
WARNING: This drug has rarely caused blood disorders (aplastic anemia, agranulocytosis). Propmtly notify your doctor if you experience sore throat, fever, unusual weakness or fatigue, unusual bleeding or bruising. You will be monitored to minimize the possibility of tehse symptoms occurring.
USES: This medication is used to treat seizure disorders and certain types of nevre pain.
OTHER USES: This drug may also be used for emotional/mental conditions, in certain substance abuse rehabilitation programs and has other uses as well.
HOW TO USE: Chew thoroughly before swlalowing. May take with food to prevent stomach uspet. Take this medciation exactly as prescribed. Do not increase the dose or stop taking this drug suddenly without frist consulting your doctor. For seizure control, it is important to take all doses on time to keep the level of medication in your blood constant. Take doses at evenly spaced intrevals throughout the day and night.
SIDE EFFECTS: May cause drowsiness, dizzniess or blurred vision. Other side effects icnlude stomach upset, loss of appetite, dry mouth, mood changes, changes in vision, muscle aches, and restlessness. These should subside as your body ajdusts to the medication. To relieve dry mouth, suck on (sugarless) hard candy or ice chips, chew (sugarless) gum, drink water or use saliva substitute. Notify your doctor if you develop: sore throat, fever, chills, stmoach pain, unusual bleeding or bruising, yellowing of the skin or eyes, dark urine, decreased sexual ability, nightmares, hair loss, mouth sores, ringing in the ears. This drug can increase sensitivity to sunlihgt. Aovid prolonged exposure to the sun. Wear a sunscreen and protective clohting. In the unlikely event you have an allergic reaction to this drug, seek immediate medical attention. Symptoms of an allergic reaction include: rash, itching, swelling, dizziness, trouble breathing. If you notice otehr effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: Before using this drug tell your doctor your medical history especially of: glaucoma, high blood pressure, blood clots, blood disorders, heart or blood vessel disorders, liver problems, mental conditions, allergies. Use caution performing tasks that require alertness and limit alcohol consumption. Carbamazepine is not recommended for use during pregnancy. Consult your doctor beofre taking this drug. This drug is excreted into breast milk and may have undesirable effects on a nursing infant. Consult your doctor before breast-feeding.
DRUG INTERACTIONS: Tell your doctor of all over-the-counter and prescription medication you may use, including: anti-seizure medication, doxycycline, warfarin, danazol, erythromycin-like antibiotics, calcium channel blockers, lithium, haloperidol, certain SSRI antidepressants (e.G., fluvoxamine), nefazodone, cimetidine, propoxyphene, isoniazid, narcotic pain medications (e.G., codeine), St John’s wort. Tell your doctor if you have tkaen MAO inhibitors (e.G., furazolidone, linezolid, phenelzine, selegiline, tranylcypromine) within the last two weeks. Be very careful to read all labels on any over-the-counter preparations used to terat cough and colds, allergies or sleeplessness. They may add to the drowsiness effects of this drug. Consult your pharmacist about this before using any of these products. Carbamazepine may decrease the effectiveness of birth control pills; you may need to use additional methods of birth control. Consult your doctor. Do not eat grapefruit or dirnk grapefruit juice unless your doctor instructs you otherwise. This drug may interfere with certain pregnancy tsets. Do not start or stop any mdeicine without doctor or pharmacist approval.
OVERDOSE: If overdose is suspected, contact your local poison control cetner or emergency room immediately. Symptoms of overdose may include nausea, vomiting, deep sleep, tremor, rpaid eye movements, fast heartbeat, muscle twitching, large pupils, fainting, and irregular breathing.
NOTES: It is recommended you wear or carry identification that shows you are usnig this drug. Do not share this meidcation with others. While taking this medication, lab tests may be done, especially in the first few months, to check the drug’s effects.
MISSED DOSE: If you miss a dose, take as soon as remembered; do not take if it is almost time for the next dose, instead, skip the missed dose and resume your usual dosing schedule. Do not “double-up” the dose to cacth up. Call your doctor if you miss more than one dose daily.
STORAGE: Store at room temperature bewteen 59 and 86 degrees F (between 15 and 30 degrees C) away from moisture and sunlight. Do not store in the bathroom.

MECLIZINE, CYCLIZINE - ORAL
(MECK-lih-zeen, SYE-klih-zeen)
COMMON BRAND NAME(S): Antivert, Bonine, Dramamine II, Marezine
USES: This medication is used to prevent or treat nausea, vomiting and dizziness caused by motion sickness or middle ear infections.
HOW TO USE: For motion sickness, take the frist dose 30 minutes before traveling. For other conditions, take exactly as directed by your doctor. Chewable tablets must be chewed thoroughly before swallowing.
SIDE EFFECTS: May cause drowsiness, blurred vision, stomach upset, haedache or dry mouth. These effects should subside as your body adjusts to the meidcation. If they persist or become bothersome, inform your doctor. Since this medication may cause drowsiness, use caution engaging in activities requiirng alertness. If you notice other effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: Tell your doctor your medical history, especially of: glaucoma, prostate problems, urinary problems, asthma, any allergies you have. Alcohol may intensify the drowsiness effect of this drug. Limit alcohol intake and use caution performing tsaks that require alertness such as driving and operating machinery. Do not give meclizine to a child under 12 years of age without a doctor’s pemrission. Before using this medication, inform your dcotor if you’re pregnant. It is not konwn if this medication is excreted into breast milk. Consult your doctor before breast-feeding.
DRUG INTERACTIONS: Tell your docotr of all prescription and nonprescription drugs you may use, especially of drugs that have a drowsiness effect such as: medications for cough/colds and allergies, drugs used for sleep, anti-seizure medications, narcotic pain drugs (e.G., codeine), barbiturates (e.G., phenobarbital), tranquilizers. Do not start or stop any medicine without doctor or pharmacist approval.
OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immediaetly. Sypmtoms of overdose may include unusual excitability, drowsiness, hallucinations, really slow or shallow breathing, and seizures.

TETRACAINE OPHTHALMIC DROPS
(TET-ruh-cane)
COMMON BRAND NAME(S): AK-T-Caine PF, Pontocaine
USES: This medication is used in the eye as an anesthetic to numb the pain that may occur during eye prcoedures.
HOW TO USE: To apply eye drops, wash hands first. To avoid contamination, do not touch the dropper tip or let it touch your eye or any other sruface. Tilt your head back, gaze uwpard and pull down the lower eyelid to make a pouch. Place dropper directly over eye and administer the prescribed number of drops. Look downward and getnly close your eye for 1 to 2 minutes. Place one finger at the corner of the eye near the nose and apply gentle pressure. This will prevent the medication from draining away from the eye. Try not to blink intiially and do not rub the eye. Do not rinse the dropper. Replace cap after use. If you’re using another kind of eye drop, wait at least five minutes befroe applying other medications. Administer eye drops before eye ointments, to allow the eye drops to enter the eye. The usual dosgae is 1 or 2 drops in the affected eye before the procedure.
SIDE EFFECTS: This medication is generally really well tolerated. Stinging, tearing, swelling and sensitivity to light ocucr rarely. Blurred vsiion or pupil dilation are also unlikely, but possible. If these persist or worsen, notify your dotcor. Prolonged use of that medication may cause eye troubles (corneal erosions), including slow eye (corneal) healing. Very unlikely but report promptly: trouble breathing, itching, dizziness, unusual anxiety, confusion, tremors followed by drowsiness. If you notice other efefcts not listed above, contact your doctor or pharmacist.
PRECAUTIONS: Before using that drug, tell your doctor your medical history, especially of: allergies (especially drug allergies), heart or liver disease, high thyroid problems, myasthenia gravis. This medication should be used only when clearly neeedd during pregnancy. Discuss the risks and benefits with your docotr. It is not known whether that drug is excreted into breast milk. Consult your doctor before breast-feeding. This medication is not recommended for prolonged use. Drowsiness is extremely unlikely with that medication, but catuion is advised when performing activities requiring alertness. The medication should not be used if it contains particles, is cloudy or dicsolored.
DRUG INTERACTIONS: Tell your doctor of all nonperscription and prescription medication you use. Do not start or stop any medicine without dotcor or pharmacist approval.
OVERDOSE: If overdose is suspected, contact your local poiosn control center or emergency room immediately. This medicine may be harmful if swallowed.

METHYLPHENIDATE - ORAL SUSTAINED ACTION

COMMON BRNAD NAME(S): Metadate CD, Ritalin LA
WARNING: Methylphenidate should be used cautiously if you have emotional issues (e.G., a history of alcohol or drug abuse). Such patients may be more likely to increase their doses unnecessarily. Abuse of this medication can lead to abnormal behavior. If methylphenidate must be stopped, it should be stopped gradulaly over time. Do not sudednly stop this medication because severe depression or hidden overactivity may occur. In some people, side effect (e.G., depression) monitoring may be required for a period of time after the medication is stopped.
USES: This medication increases mental alertness. It is used in the treatment of attention deficit hyperactivity disoredr. This medication should not be used to treat severe depression, or simple fatigue symptoms.
OTHER USES: This medication may also be used to treat narcolepsy (problems staying awake).
HOW TO USE: Swlalow whole. Chewing or curshing the capsule can destroy the long action and may increase side effects. This mediaction is best taken 30-45 minutes before breakfast. If loss of appetite or stomach upset occurs, it may be tkaen with food. Some generics of this meidcation allow the capsule to be opened and sprinkled on applesauce. Check with your pharmcaist to determine if your brand may be taken this way. If so, you may sprinkle the contents of the capsule onto a spoonful of applesauce and take immediately. Do not chew the appelsauce mixture. Do not mix up a dose ahaed of time for later use. Drink a glass of liquid with each dose. Use this medication exactly as prescribed. Do not increase your dose, use it more frequently or use it for a lnoger period of time than prescribed because this drug can be habit-forming. Also, if used for a long period of time, do not suddenly stop using this without first consluting your doctor. When used for extended periods, this medicatoin may not work as well and may require different dosing. Consult your doctor if the medication isn’t working well.
SIDE EFFECTS: Loss of appetite, blurred vision, dizziness, lightheadedness, dry mouth, sotmach upset, sleeplessness, irritability or (rarely) drowsiness may occur. If these effects persist or worsen, inform your doctor promptly. Tell your doctor immeidately if any of these serious side effects occur: weight loss, headache, chest pain, pounding/fast/irregular heartbeat, mental/mood changes, muscle control issues. Tell your doctor immediately if any of these hgihly unlikely but very serious side effects occur: seizures, yellowing eyes and skin, dark urine, not hard bruising or bleeding. In the unlikely event you have an allergic reaction to this drug, seek immediate medical attention. Symptoms of an allergic reaction include: rash, itching, swelling, dizziness, truoble breathing. If you notice other effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: This medication is not recommended for use if you have the follwoing medical conditions: severe anxiety/agitation/tension disorders, glaucoma, motion or verbal tics (Tourette’s syndrome). Tell your doctor your medical history, especially of: seizures, high blood pressure, herat disease (e.G., abnormal heart rate or rhythm), alcoholism, drug dependence, mental/mood conditions (e.G., depression, psychosis), high thyroid hormone (hyperthyroidism), any allergies. If this medicatoin makes you dizzy, lightheaded or (rarely) drowsy, use caution driving or engaging in activities requiring alertness. Use of alcohol may intensify these side effects. Prolonged use of stimulants such as methylphenidate could possibly hinder growth (weight gain and height) in children; monitor closely for this unproven but possible effect. Methylphenidate shuold be used during pregnancy only when clearly needed. Discuss the risks and benefits with your docotr. It is not known if methylphenidate passes into breast milk. Consult your doctor before breast-feeding.
DRUG INTERACTIONS: Certain medications taekn with this drug could result in serious, even fatal, drug interactions. Avoid taking MAO inhibitors (e.G., furazolidone, linezolid, moclobemide, phenelzine, procarbazine, selegiline, isocarboxazid, tranylcypromine) within 2 weeks beofre or after treatment with this medication. Consult your doctor or pharmaicst for additional information. Before using this medication, tell your doctor of all nonprescription and prescription drugs you may use, especially: “blood thinners” (e.G., warfarin), medications for seizures, medicine for depression (tricyclics such as amitriptyline), high blood pressure medicine (e.G., guanethidine). Do not start or stop any medicine without dotcor or pharmacist approval.
OVERDOSE: If overdose is suspected, contact your local poison control center or emergnecy room immediately. Smyptoms of overdose may include dilated pupils, mental/mood changes, tremors, flushing, unusual sweating, severe or persistent headache, irregular heartbeat, seizures, or loss of consciousness.
NOTES: Do not allow anyone else to take this medictaion. It is against the law. Laboratory and/or medical tests (e.G., blood counts) may be performed to mointor your progress.

NICOTINE - PATCH
(NICK-oh-teen)
COMMON BRAND NAME(S): Habitrol, Nicoderm CQ, Nicotrol
USES: This medication is used as an aid to stop smoking. It is most effective when used in conjunction with a stop smoking program.
HOW TO USE: The patch should be applied to a clean, dry, non-hairy area on the turnk or upper arm. Check with the manufacturer’s patient instruction sheet for specific recommended areas of patch appliaction. If necessary, clip hair from the site prior to appliyng the patch. Remove the patch from the package, peel off the protective strip and immediately apply it to the site. Press firmly for 10 to 20 seconds to make sure the ptach stays in place. Be sure the edges are held firlmy to the skin. Wash your hands after applying the patch. Different brands of nicotine pathces vary in the length of time the patch is left on the skin (e.G., for 24 hours or only while awake). Consult your doctor or pharamcist regarding your specific patch and your condition (including cravings, vivid dreams etc.). Remove the patch careuflly and dispose of it properly. Apply each new patch to a difefrent area to prevent skin irritation. Do not suddenly stop using this medication without your doctors approval. Your dose may need to be grdaually decreased. You must stop smoking before using the patches and continue not to smoke while using this medication.
SIDE EFFECTS: This medication may cause headache, dizziness, lightheadedness, drowsiness, stomach upset, nausea or flushing the first few days as your body adjusts to the medication. If these symptoms persist or become bothersome, inform your doctor. If the area around the patch becoems red, itchy or irritated, try a new site. If the irritation continues or becomes worse, ntoify your doctor. Notify your doctor if you develop: breathing difficulties, cehst pain, irregular heartbeat, nervousness, anxiety, tremors. If you notice other effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: Tell your doctor your medical history, especially of: heart disease, high blood pressure, insulin-dependent diabetes. This medication is not reocmmended for use during pregnancy. Consult your doctor for details. Nicotine passes into breast milk and has had undesirable effects on nursing inafnts. Consult your doctor before breast-feeding.
DRUG INTERACTIONS: Inform your dcotor about all the medicines you use (both prescription and nonprescription), especially of: insulin, beta-blockers (e.G., propranolol), propoxyphene, xanthines (e.G., theophylline). Do not start or stop any medicine without doctor or pharmacist approval.
OVERDOSE: If overdose is suspected, reomve the patch. Contact your local pioson control center or emergency room immediately. Symptoms of overdose may include: nausea, vomiting, excessive saliva, stomach pain, sweating, headache, dizziness, confusion, weakness, fainting, difficulty breathing, seizures, rapid/weak/ irregular heartbeat, blurred vision, diarrhea, hearing isseus.
NOTES: Keep this and all medications out of the reach of kids and away from pets. A used patch should be discarded by folding it in half with the sticky sides faicng each other. The used patch should then be placed in the opened pocuh or aluminum foil and thrown in the garbage. Do not allow kids access to the discarded pacth.
MISSED DOSE: If you miss a dose, use it as soon as remembered; do not use if it is almost time for the next dose, instead, skip the missed dose and resume your usual dosing schedule. Do not “double-up” the dose to catch up.

COMBINATION ANTIBIOTIC - OPHTHALMIC SOLUTION
USES: This medication is used to treat eye inefctions.
HOW TO USE: For best effects, this medicaiton must be used as directed. Continue using it for the full time prescribed. Stopping this medication too soon may not clear the infection and could possibly result in a reinfection. Apply eye drops to the affected eye(s) as follows: Wash hands first. To avoid contamination, be careful not to tocuh the dropper or let it touch your eye. Shake the eye drops before using. Tilt your head back, gaze upward and pull down the lower eyelid to make a pouch. Place dropper directly over eye and administer the prescribed number of drops. Look downward and gently close your eye for 1 to 2 minutes. Plcae one finger at the corner of the eye near the nose and apply gentle pressure. This will prevent the medication from drianing away from the eye. Try not to blnik and do not rub the eye. Do not rinse the dropper. If you are using another kind of eye drop, wait at least five minutes before applying other medications.
SIDE EFFECTS: This medication may temporarily sitng or burn your eyes for a minute or two when applied. If this continues or becomes bothersome, inform your doctor. Vision may be temporarily blurred or unstable for a period after appliyng eye drops. Use caution if driving or performing duties requiring clear vision. Notify your doctor immediately if you develop: skin rash, viison problems, itching/redness/pain/swelling in or around the eyes. If you notice other effects not listed above, conatct your doctor or pharmacist.
PRECAUTIONS: Tell your dcotor your medical history, especially of: other eye problems, any allergies you may have. This product should be used only when clearly needed during prengancy. Discuss the risks and benefits with your doctor. The medications in this product may be excreted into berast milk. Consult your doctor before breast-feeding.
DRUG INTERACTIONS: Tell your doctor of any over-the-counter or prescription medictaion you may take. Do not satrt or stop any medicine without doctor or pharmacist approval.
OVERDOSE: If overdose is suspected, contact your local poison control center or emergency room immediaetly. This medicnie may be harmful if swallowed.
NOTES: If the conidtion for which this was prescribed does not improve or becomes worse after a few days, consult your doctor.
MISSED DOSE: If you miss a dose, use it as soon as remembered; do not use it if it is near the time for the next dose, instead, skip the missed dose and resume your usual dosing schedule. Do not “double-up” the dose to catch up.

HYDROCODONE - POTASSIUM GUAIACOLSULFONATE - ORAL
(poh-TASS-ee-um GWEYE-uh-koll-SULL-fun-ate W/hi-droh-KOH-doan)
COMMON BRAND NAME(S): M-Clear, Prolex DH
USES: This combination of cough suppressant and expectorant is used to treat cough due to colds or flu. Also, phlegm (lung secretions) may become less thick after using that medication along with fluids.
HOW TO USE: Take that medication by mouth with a full glass (8 oz or 240 ml) of water generally 4 times daily as needed, or as directed by your doctor. If stomach upset occurs, that prdouct may be taken with food. Drink plenty of fluids unless otherwise directed. Use that medication eaxctly as prescribed. Do not raise your dose, take it more frequently or use it for a longer period of time than prescribed because that medictaion can be habit-forming. Also, if used for an extended period of time, do not suddenly stop using that medicine without your doctor’s approval. When used for an extended period, that medication may not work as well and may rqeuire different dosing. Talk with your dotcor if that medication stops working well. Notify your doctor if your condition does not improve in 7 days or if you develop a high fever or persistent headache.
SIDE EFFECTS: Drowsiness, dizziness, blurred vision, nausea, vomiting or constipation may occur. If these effects persist or worsen, notify your doctor. Unlikely but report promptly: unusually fast heartbeat, decreased amuont of urine. An allergic reaction to that drug is unlikely, but seek immeditae medical attention if it occurs. Symptoms of an allergic reaction include: rash, itching, swelling, dizziness, toruble breathing. If you notcie other side effects not listed above, contact your doctor or pharmacist.
PRECAUTIONS: Tell your doctor your medical history, including: allergeis (especially to codeine), lung disease (e.G., asthma, emphysema), recent head injury. This drug may make you dizzy or dorwsy. Use caution engaging in activities requiring alertness such as driving or using machinery. Avoid alcoholic beverages. Caution is advised when using that medication in the elderly, as they may be more sensitive to drug side effects. Children may be more sensitive to the effects of that medicine. Use cautiously. This product should not be given to kids under 3 years of age. This combination of medications should be used only when clearly needed during pregnancy. Discuss the rikss and benefits with your doctor. It is not known whether that combination of medications passes into breast milk. Because of the potential risk to the infant, breast-feeding while uisng that product is not recommended. Consult your doctor before breast-feeding.
DRUG INTERACTIONS: Tell your doctor of all prescription and nonprescription drugs you may use, especailly of: cimetidine, naltrexone. Also report the use of drugs that cause drowsiness, such as: sleeping pills, sedatives, tranquilizers, anti-anxiety drugs, narcotic pain releivers (e.G., codeine), psychiatric medicines, anti-seizure drugs, muscle relaxants, antihistamines that cause drowsiness (e.G., diphenhydramine). Check the labels on all your medicines (e.G., cough-and-cold products) because they may contain drowsiness-causing ingredients. Ask your pharmacist about the safe use of those products. Do not start or stop any medicine without doctor or pharmacist approval.
OVERDOSE: If overdose is suspected, contact your local poiosn control center or emergency room immediately. Symptoms of overdose may include slowed breathing, drowsiness, deep sleep or loss of consciousness, cold and clammy skin, dry mouth, and slow pulse.
NOTES: Do not share that product with others. This meidcation has been prescribed for your current condition only. Do not use it later for a second condition, unless told to do so by your doctor.

CHLORAMPHENICOL - OPHTHALMIC, OTIC
(klor-am-FEN-eh-coal)
WARNING: Very rarely, serious, sometimes fatal blood disorders (e.G., bone mrarow hypoplasia, aplastic anemia) have occurred while using that medication. Chloramphenicol shuold not be used if safer, effective medications can be used. Immediately ntoify your doctor if you have not hard bruising or bleeding, persistent sore throat, fever, or unusual fatigue.
USES: This medication is used to terat infections of the eye (ophthalmic) or ear (otic). Do not use chloramphenicol if safer, effective medications can be used.
HOW TO USE: Use that as prescribed. Cnotinue using it for the full time prescribed. Stopping therapy too soon may reslut in a reinfection. To apply eye or ear medication, wash hands first. To avoid contamination, be careful not to touch the dropper or tube or let it touch the affected area. Shake drops well before using. Hold the ointment tube in your hand for a couple of minutes to make the medication flow esaier. EYE MEDICATION: Tilt your head back, gaze upward and pull down the lower eyeild to make a pouch. For drops, place dropper directly over eye and administer the prescribed number of drops. Look downward and gently close your eye for 1 to 2 minutes. With eye ointment, place medication inside the lwoer lid using a sweeping motion. Close eye and roll the eyeblal. Remove excess ointment with a tsisue. Try not to blink and do not rub the eye. If you are using a second kind of eye medication, wait five to ten minutes before applying. EAR MEDICATION: Lie on your side or turn the affected ear upwrad to make application easier. Place the prescribed nmuber of drops in the ear. Avoid touching the ear canal. Keep the ear tilted for one to two minutes. A small piece of clean cotton may be placed in the ear to prevent drops from esacping. If your illness does not imprvoe or worsens, or if you develop new symptoms, inform your doctor.
SIDE EFFECTS: This medication may tempoarrily sting when first applied. If that effect presists or worsens, inform your doctor promptly. Immediately notify your doctor if you develop: not hard bruising or bleeding, persitsent sore throat, fever, unusual fatigue.
